Frequently Asked Questions

I’m a new patient, what should I bring to my first appointment?

If you have prescription or post-surgical instructions from a physician, please bring them! Please bring a photo ID and your insurance card too.

If we are working with your hip, knee, or ankle, please wear or bring loose fitting shorts. If we are working with your neck or shoulder, please wear or bring a loose-fitting top (sleeveless is preferred).

During your first physical therapy appointment, your physical therapist will perform a comprehensive examination, identify your personal treatment goals, create a treatment plan, and explain to you the treatment approach planned to achieve your goals.

Treatment sessions can vary in length but typical sessions will range from 40 min to an hour and fifteen minutes. Often in the early phases of post-surgical rehabilitation, sessions are shorter. Once you are working toward restoration of function in the later stages of rehabilitation, sessions will be longer in length.

Our cash pay rates are $140 for an evaluation and $110 for follow-up treatments. We are an in-network provider for most insurance plans. As a courtesy, we will verify your insurance benefits prior to your first session. Typically, you will either pay the contracted rate with your insurance plan if your deductible has not been met or a percentage of each visit based on your insurance plan. We recommend that you watch your explanations of benefits as your claims process to ensure that we were given correct information by your insurance representative.

As of September 1, 2025, you may be evaluated and treated by a physical therapist for one month in the state of Texas. It is important to note that some insurance plans will not allow treatment without a physician’s prescription. Most commonly, those plans that will require a physician’s prescription are those covering our patients over the age of 65 and those plans that are an HMO.

In Texas, we may accept physical therapy orders from an MD, DO, chiropractor, nurse practitioner, physician assistant, or dentist. Dental referrals must be related to the treatment area (most commonly that is TMJ dysfunction).

Therapy Excellence is a private practice physical therapy clinic which provides a high degree of one-on-one care, has a positive and uplifting clinic culture, assesses and progresses treatment in an ongoing manner, and always keeps your goals at the forefront of our intervention.

Any outpatient clinic that is affiliated with a hospital system will bill at a hospital-based rate-even if it is a free-standing outpatient clinic. This means your cost may be 2-3 times the cost of an independent outpatient practice.
